Job Summary: the Visiting Nurse is responsible for the provision of care to clients and the supervision and mentoring of Caregivers (Home Care Aides).

Major Responsibilities/Activities:

o Provision of Services to Clients:

• Conduct in-person assessments for Personal Care Services clients, including developing a plan of care in conjunction with client's needs.
• Conduct 30 day status update calls and 60 day in-person re-assessments to assigned clients to ensure quality of care provided and client satisfaction with services.
• Complete required documentation for all initial assessments, 30 day calls and 60 day re-assessments, as well as care plan reviews with Caregivers, any phone or email correspondence with clients or family members.
• Monitor and respond to voicemails and email messages related to assigned clients.

o Supervise licensed Certified Home Health Aide performance

• ·Support and mentor Caregivers and provide ongoing supervision and training to ensure client satisfaction.
• Assist Client Services Administrator and/or owner with selection of appropriate caregivers for clients based on skill level and care required.

o General Activities

• Maintain positive relationships with all Caregiving staff, clients, family members, referral sources and other organizations in the conduct of SHS business.

Minimum Requirements:

· Active NJ RN license

· Minimum 3 years med/surg experience in hospital setting.

· Prefer 2 years home care experience.

Knowledge, Skills and Abilities:

1. Requires strong clinical knowledge and skills to assess client needs.

2. Requires strong communications skills and problem solving skills and the ability to prioritize time.

3. Requires the ability to build relationships with customers and staff.

4. Requires strong organizational and documentation skills including attention to detail.

5. Requires supervisory skills.

6. Requires strong computer, intranet and social media skills.

Job Type: Part-time

Pay: $38.00 - $42.00 per hour
