Our mission

Puente fosters wellness and prosperities in the San Mateo County South Coast communities of Pescadero, La Honda, Loma Mar, and San Gregorio by promoting and advocating for equitable access to education, health, and economic security.

Position Summary

The Senior Services Associate is responsible for providing day-to-day case management and connecting older adults in Pescadero, Loma Mar, La Honda and San Gregorio to Puente services and other appropriate program referrals to reduce social isolation, loneliness, promote health and connection.

Responsibilities

Community Outreach and Program Referrals:

· In coordination with health promoters, regularly engage in home visits to program participants, local farms, and trailer park.

Program Planning

· With support from the Community Health Director, develop appropriate program activities for senior South Coast residents which includes organizing monthly events related to health, education, and economic security relevant to this population.

· Establish an evaluation method that accurately reflects the successes and failures of Puente’s older adults’ programs in the community.

· Work with locals and other community-based organizations to create inter-agency coalitions that aim to promote the health of older adults on the South Coast.

Data Collection:

· Collect and record data in a timely manner, including attendance for program activities and events, outreach efforts, meetings, and interactions with partner agencies. Identify program outcomes.

Other responsibilities:

· Establish positive collaborative relationships with appropriate community organizations, and related agencies throughout the region.

· Attend weekly team and staff meetings as well as Puente-sponsored community events. Manage a case load of up to 18 participants at a time.

Qualifications

· Bachelor’s degree in social work, human services, gerontology, counseling or related field or a combination of education and experience.

· Passionate about working with older adults who may be isolated, living with mental illness, cognitive impairments who may have additional unmet needs.

· Experience in case management, program planning, and providing direct services to program senior program participants.

· One year experience working with senior population.

· Experience in community organizing, and/or health advocacy.

· Ability to organize tasks and people in an efficient manner.

· Proficient in Microsoft Office products, including Word, Excel, Outlook, and PowerPoint.

· Commitment to Puente’s vision of supporting a healthy, inclusive, and sustainable South Coast community.

· Works collaboratively with colleagues, partners, and community members.

· Excelled verbal and written communication skills in English and Spanish.

· Cultural competency to work with residents from diverse backgrounds.

· Must possess current California Driver’s License and ability to drive to work-related functions in and around Southern San Mateo County.

· Knowledge of the South Coast or experience working in a remote community, preferred.

· Current California Driver’s License and ability to drive for work related activities, mostly in the South Coast and San Mateo County

Salary: $25.00 - $28.75
