Position: RN - Resident Care Coordinator

Job Summary:

Supervises and coordinates care of assigned Residents, compliant with Company Policies, State, Federal Laws and Regulations; must exhibit sound judgment, have a sincere desire to work with elderly; must communicate courteously and respectfully with Residents, families, Employees and others

Qualifications:

RN Nursing License in good standing in State of Mississippi; must attain TB Certification, QAPI Certification; minimum two years’ experience in Long-Term Care with thorough knowledge of principles and practices of Long-Term Care, knowledge of State and Federal Regulations for Long-Term Care; must pass drug screens and have a satisfactory Criminal History Background Check

Duties and Responsibilities:

  • Assists RN Supervisor and Resident’s Primary Nurse in the Admissions Process
  • On Admission and/or within next few days, explains to Resident and family what our goals for the Resident are; explains our routines and Care Guidelines to Resident and family
  • Follows up with Resident’s family during the first week, then weekly within next two weeks, keeping family informed; documents communication with family
  • Discusses new Resident with DON to determine acuity level (high-risk areas and quality of care issues); room assignments; determines prior to Admission Resident’s needs such as specialty bed etc.
  • Encourages Resident and family regarding Resident’s care as appropriate; invites family to Family Council Meetings and informs Residents of monthly Resident Council Meetings
  • Reviews medical records for acute situations such as, Medicare Part-A documentation, I /A’s and new orders
  • Reviews medical records for compliance with care plans and other paperwork within 24 hours of admission, excluding weekends and holidays
  • Reviews medical records of Residents with wounds at least weekly for measurements, weights, labs and any interventions needed, such as specialty beds, turning, repositioning
  • Reviews documentation of prevention interventions for Residents at high-risk for falls, skin breakdown, weight loss, dehydration, elopement and infections,; discusses changes with Physician, Resident and family
  • Reviews all medical records for documentation errors
  • Reviews medical records for documentation that supports the care plan, such as Resident’s non-compliance or behavior problems; documentation should not be derogatory or negative but should only State observations
  • Reviews medical records to ensure that care plan supports the acceptable standard of performance with the medical diagnosis made by the physician
  • Reviews medical records to ensure all Resident and family complaints are documented and investigated; follows up to ensure concerns are resolved and maintains availability to Resident and family
  • Attends Care Plan Conferences as requested
  • Provides in-services to families in specific areas of concern
