Job description

· Provides consultation and direction to ensure programs and services are implemented at the highest standards to ensure the health and safety of all clients, at the highest level of care.

· Maintain RN License with continuing education hours as per the State Nursing Board Requirements, including continued education, reading leadership manuals, and books to acquire up to date knowledge in their field.

· Invited to attend weekly PCP meetings, as needed.

· Invited to Attend CPOC meetings and Client’s doctors’ appointments when requested.

· Make recommendations to management concerning such issues as procedural changes.

· As needed, teach the 16-hour Medication Administration Class according to La State Regulations and SCUSA Policy & Procedure.

· Complete certificates once a DSW has met the medication administration training requirements, including documentation that the RN conducting the training has assessed the proficiency and determined that the DSW exhibits sufficient proficiency to be able to administer medications safely and/or perform non-complex tasks safely.

· Complete and document initial Nursing Assessments.

· Complete and document Annual Nursing Assessments for current clients within 365 days of last assessment. (Provide advance notification when not available for completion)

· Complete and document required post hospital assessments within 3 to 5 days after discharge. (Provide notification upon assessment request if not available to complete)

· Visit medically needy clients in their home environment, as needed, at the request of the Case Manager, Field Supervisor, Administrative Team, and/or other client team members.

· Complete periodic client assessments based on client’s health and changes in Physician’s Order.

· When available, Retrain DSW’s when a medication error occurs and provide all documentation to the training and medical depts with in the time line indicated in the policy and procedures.

· Perform Medical Client Specific trainings for any DSW in the home environment of the client for medication administration and will retrain should the client’s health status change or physician’s order change.

Provide additional person-specific training when the client receiving care has a change in health status or physician orders and yet remains in a stable predictable condition.

Job Types: Part-time, Contract

Pay: $20.00 - $30.00 per hour
