Compass Health is seeking qualified Patient Care Coordinators to join us in our mission to improve lives by passionately serving others.
The Patient Care Coordinator works interdependently with the eCare multidisciplinary team to maximize quality outcomes for individuals. The Patient Care Coordinator serves as the primary point of contact with clients in the client’s home.

The Patient Care Coordinator assists the multidisciplinary team in assessing clients' needs, situations, strengths, and support networks to determine their goals. Responsibilities include performing in-home visits, information gathering, and monitoring and support of equipment use. May at times, assist and perform other functions as requested. This position reports to the Program Director.
Requirements include:
- Must possess oral and written communication skills.
- Must possess a valid Louisiana driver license and reliable transportation.
Skills:
- Able to communicate effectively in English, both verbally and in writing.
- The ability to adapt to the organization policy and procedural changes.

- Honesty and integrity in all aspects of the day-to-day activities.
- Ability to use Microsoft Office365.
- Ability to work with little to no supervision, manage own schedule, prioritize, and perform multiple tasks effectively.
- Implement critical thinking in assessing and resolving complex client-related crises.
- Ability to drive to patients' homes and assist with the set-up of the equipment they will be utilizing for their sessions.

- Lake Charles
- Surrounding areas
- MUST be able to drive on highway
Schedule:
- 8 hour shift
- Monday to Friday
