Consider joining our team as an AmeriCorps Community Health Worker with the Epilepsy Foundation New England (EFNE). We're currently seeking candidates to connect individuals and families living with epilepsy with social and community resources and help them navigate their healthcare journey. This position offers paid service training leading to a Community Health Worker certification. As an EFNE Community Health Worker, you'll be managing an Epilepsy Resource Room, which serves as a valuable hub for patients seeking information and support. You'll work with a talented team at EFNE and a major medical institution, providing referrals to age-appropriate EFNE and hospital-based programs and resources to increase health knowledge for patients and families living with epilepsy. With flexible hours and a hybrid work environment, you'll have the opportunity to make a real impact in the lives of those living with epilepsy. This 11-month service program begins in June 2023. Primary Duties and Responsibilities:

  • Provide active listening, support, and coaching to patients on life changes that support health and wellness.
  • Build community among patients by developing and implementing new and existing youth programs.
  • Connect patients to EFNE, hospitals, and community resources.
  • Arrange inpatient support and/or recreational programs.
  • Assist in new initiatives as well as participate in interventions, telehealth, resource referrals, and back-to-school support.
  • Mentor patients and connect them to community resources.
  • Implement community outreach by participating in events and providing education.
  • Maintain a library of helpful literature while assisting hospital staff with site-specific needs to improve patient support.
