Job Description

• Under the direction of the RN, assists in identifying the patient’s physical and environmental needs as evidenced by documentation.

• Participates in planning and implementing care in partnership with the patient, family, caregiver, and RN. Provides services that are ordered by the physician as indicated in the plan of care.

• Provides patient, family, and caregiver education.

• Provides effective communication to the patient, family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes.

• Completes appropriate documentation within 24-48 hours.

Requirements For Qualified Candidates

• Graduate of accredited practical/vocational nursing program

• Currently Licensed Practical/Vocational Nurse in the state(s) of agency operation

• Must possess and maintain valid CPR certification while employed in a clinical role

• One (1) year of clinical experience

• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
