Job Opening Summary
Inform, recruit, consent and enroll eligible individuals implemented at medical practices. Work closely with healthcare providers to ensure continuity of individuals in the program and administer survey and interviews at pre-determined collection points.
Job Opening Qualifications
High School Diploma or GED
:
Required Licensure/Certifications:
- Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross
- Community Health Worker Certification (CHCW) from the Florida Certification Board within one (1) year of hire or transfer into the position.
- Level II Background clearance is required.
- Valid Florida or Georgia Driver's license with clean record, or violations not exceeding maximum points as provided in hospital policy.
Preferred Licensure/Certifications:
Certified Community Health Worker (CCHW) from The Florida Certification Board
Required Experience:
Less than 1 year healthcare related experience
Preferred Experience:
N/A
Necessary Skills:
1. Strong written and verbal communication skills
2. Advanced Beginner in Microsoft Word, Excel, and Outlook
