Postion Summary

This position will provide case management and care coordination services, as part of a care coordination team, to low-risk pregnant women, their infants and families, and/or fathers/male partners in assigned service areas in any of the following counties: Bolivar, Coahoma, Quitman, Sunflower, Tallahatchie, Tunica, and Washington. Coordinating health care services, referrals, and community resources; collecting data, and reporting are the primary responsibilities for this position. The Community Care Coordinator will implement individualized plans of care, mutually designed by each client and licensed care coordinator. This is a full-time position. This job requires travel.

Experience/Qualifications

A minimum of one year toward completion of an associate’s degree in a directly related field is required, but a bachelor’s or an associate’s degree in a directly related field from an accredited institution of higher learning is preferred. Maintaining and submitting accurate, complete, and timely documentation and effectively presenting information to community groups and school groups is required. The incumbent must demonstrate a genuine concern for women and children in the community. Strong communication and leadership skills and the ability to work with diverse populations, including clients, local health care providers, community leaders, community volunteers, school officials, local/state/federal officials, and media is also required.
