CNS is looking for a Clinical Social Worker for Salt Lake and Tooele County.

The social worker will perform home visits for home-health, hospice and palliative patients and their families. Assist patients and families to understand, accept and follow medical recommendations. Aide families in the utilization of family and community resources as indicated. Assist the multidisciplinary team to understand the social and emotional factors, and participate in patient treatment plans.

Responsibilities:

• Assessing emotional factors related to terminal illness; assisting the physician and other team members in understanding these factors;
• Assessing the patient/caregiver psychosocial status;
• Assessing the patient’s potential risk of suicide and/or potential for abuse or neglect;
• Assessing environmental resources and obstacles to maintaining safety;
• Participating in the development and revision of a plan of care;
• Providing social services including crisis intervention, assist in providing information and preparation of advance directives, funeral planning, issues involving transfer of fiscal, legal and health care responsibility;
• Preparing clinical and progress notes;
• Identifying family dynamics, communication patterns, and involving the family/significant other in the plan of care;
• Identifying and utilizing appropriate community resources and assessing patient/family/caregiver’s ability to access them;
• Participating in inservice programs;
• Assessing caregiver’s ability to function adequately;
• Assessing need for counseling related to risk assessment for pathological grief and refer;
• Assessing special needs related to cultural diversity including communication, space, role of family members, and special traditions;
• Identifying the developmental level of patient/family/caregiver and obstacles to learning or ability to participate in care of patient;
• Addressing patient/family questions and issues

Education/Experience/Licensure/Certs/Permits:

• Bachelors of Science with degree of social work (BSW).
• Licensed Social Service Worker preferred. Must be a licensed social worker in Utah.
• Prefer at least one year of hospice experience, must have two years of home care experience.
• Valid driver’s license and auto insurance.
• BSW to be provided supervision through CNS or by other approved arrangements.

Pay: $42,072.31 - $87,352.29 per year
