Schedule: 4- 10 hour shift with every other weekend

Reviews orders and services as appropriate, including add-on disciplines, according to the clinician's initial client assessment.

  • Ensure the accuracy and legal compliance of orders by the clinician
  • With collaboration of the field clinician, Edit/approve clinician schedule as applicable to meet plan of care
  • Review client medication profile and schedule
  • Function as a resource for clinicians, internal staff and physicians
  • Receive and respond to calls/notifications regarding client charts
  • Collaborate with clinicians as necessary should the order need to be edited
  • Document information regarding calls/notifications into the Coordination Notes and update personnel as needed to ensure proper coordination of care
  • Review utilization of services based on the documentation provide by the clinical team
  • Maintain a professional appearance as a representative of the company
  • Read and adhere to all Agency Policies and Procedures and follow Employee Handbook Guidelines
  • Complete and submit all required documentation within specified company requirements
  • Actively promote the values, mission and culture of the Company
  • Other Duties as assigned

Qualifications: • Licensed Practical Nurse (LPN) with a current license in the state of Ohio

  • Maintain licensure as practicing Clinician per the appropriate Ohio Board
  • A minimum of two (2) years’ experience in the health care industry; Home health experience preferred
  • Attend in-service trainings and mandatory agency meetings
  • Knowledge of guidelines governing home health agencies
  • Knowledgeable on clinical best practices. Ability to make appropriate judgments
  • Ability to follow directions and work as a team member
  • Disciplined style of work ethic
  • Ability to prioritize and organize/ Works efficiently and timely
  • Dedication to process excellence and best in class in readmission rate
  • Ability to create positive impressions and communicate with a variety of people
  • Maintain effective communication with patients, families, physicians and co-workers

The Clinical Review Specialist (CRS) is a LPN or RN responsible for the overall coordination of the day-to-day clinical workflow. The CRS is responsible for the provision of quality services according to acceptable standards of practice and continually monitors the services provided. A resource person with extensive knowledge of the regulatory guidelines governing home health agencies and complies with all agency policies and legal rules and regulations.
