How will you impact your clients’ lives?
  • You’ll be using your expertise to help your clients set their mental health goals, for example, getting housing, going back to school, finding a primary care doctor, etc.
  • You’ll engage in care activities to empower your clients to help them achieve their goals.
  • Applying your tech guru skills, you’ll collect and enter data into our case management systems, such as Relevant, so we can keep track of clients’ progress.
  • You’ll fight for your client's rights! As an advocate for your client, you will be the voice on their side in the battles they can not fight alone. Whether it is working with The Department of Social Services to figure out how to handle their rent arrears or even asking landlords to extend a client’s lease.

We want to trust that you can manage your caseload like your own agency by making proper decisions for clients’ but when in doubt always seek help. With this work, you are offered a great deal of independence therefore accountability is a must.
Essential Functions of the Role:
  • Supervise, train, and develop Client Care Coordinators in their ability to address their client’s immediate and emerging needs, resolve issues, and advocate and connect clients to needed resources such that their lives become stable, recovery-centric, and interactive (minimizing isolation).
  • Monitor, review, and verify monthly Medicaid billable services that Case Managers provide their caseloads.
  • Lead and coach assigned teams in a manner they become increasingly independent in their capability to manage day-to-day interactions by developing their problem-solving, communication, and advocacy skills; as well as providing them with needed training (one-on-one, field visits, team meetings, outside training, etc.) that will result in effective and outcome-oriented case managers.
  • Ensure care plans, activities, and required documentation are aligned with client goals and in compliance with Health Home or Grant Funding requirements; eliminate potential issues related to billing, reimbursements to the agency, and/or an audit from an external entity.
  • Be aware of and provide guidance, and/or direct involvement in cases presenting with acute needs.
  • Conduct supervision meetings with each ICM (one-on-ones) reviewing key metrics
  • Comply with all other reporting requests made by management.
  • Monitor ICM quality through direct calls to clients, field visits, etc., side-by-side.
  • Develop productive and results-oriented relationships with mission-critical persons at outside organizations, such as (but not limited to): the Dept. of Social Services, Mental Health Providers, Primary Care Physicians, Probation Officers, Legal Services, District Attorney’s Office
  • Team and location assignments based on Agency needs.
  • Other activities as assigned.
