Do you have a background in geriatrics care as a Nurse, Social Worker, or Care Manager? We are looking for a Client Care Manager for our Home Health, non-medical Agency, Senior Helpers. We provide quality care for elderly clients and their families by providing dependable and affordable care. As the Client Care Manager you will go beyond merely giving our clients what they want; you will help solve problems, analyze markets, and help us achieve our business goals by allowing seniors to stay in their homes. Compensation : Depends on experience. Base salary + quarterly bonuses + commission. PTO and paid health coverage

Job Responsibilities: (Including but not limited to):

· Take inquiry calls and follow up on all prospects leads to achieve organization’s goal

· Perform Client Assessments, by creating a care plan for each client and make changes as warranted.

· Contact clients on regular basis to receive feedback to ensure the service is meeting the goals outline in the care plan. Ensure Clients and their families are pleased with Senior Helper’s service.

· Respond promptly to any client complaints, documents complaints, and review complaints with Director of Operations.

· Perform on-going reassessments with clients and/or family and recommend required changes in care plans.

· Work closely with Client Services to plan care for clients and fill shift vacancies.

· Maintain accurate client information in software management systems.

· Build and nurture relationships with other Home Health and Hospice Agencies.

· Possess the ability and skill to “close” sales and execute contracts.

· Other duties as assigned

Qualifications:

Minimum 3-year experience in similar position or experience in geriatric care and/or social services for the elderly.

Team player, excellent verbal, and communication skills, adaptable in different situations, possesses excellent client interaction skills, able to multi-task and work independently.

Outstanding customer service skills.

Must possess a valid driver's license, reliable transportation, and current automobile insurance.

Job Type: Full-time

Pay: $48,000.00 - $65,000.00 per year
