We are a local family-owned home care business in Norfolk, Nebraska that has an exceptional culture, and we are looking for a Client Care Coordinator to join our AWESOME team! We offer PTO, Paid Holiday's, 401k, employee discounts, and flexible schedule! GREAT HOURS! The hours are 8am-5pm Mon-Friday.

Great Base Pay plus Bonuses!

The Client Care Coordinator is responsible for creating extraordinary client and family experiences and ultimately manages “best in-home care” care plans.

  • Conducts Home Assessments and aides in developing, modifying, and maintaining the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.
  • Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided by conducting regular Supervisory Visits Announced and Unannounced.
  • Serves as a mentor and coach to caregivers and supports their care delivery.
  • Oversees the Quality of Care being provided in the field and passes along to the Recruiters positive and constructive feedback that was evaluated during the visits.
  • Conduct periodic Audits to ensure compliance documents are up to date for our Care Staff. (Ensure Driver’s License, Car Insurance, Professional Licenses, and Care Logs are up to date)
  • Work with Recruiters on any clinical issues/concerns that may warrant disciplinary action up to termination.
  • Conduct Caregiver/Client Introductions as requested.
  • Solicit Client feedback/reviews based on the client experience.
  • Conduct Random Drug Testing periodically during visits.
  • Any downtime can be spent helping with Scheduling/Recruiting.
  • Split time in the office and traveling to client’s homes. Company vehicle provided.

The desired candidate will possess the following:

  • High school graduate.
  • Active CNA or LPN/RN license in the state of Nebraska.
  • Bilingual STRONGLY Preferred.
  • Able to work independently, demonstrating sound judgment.
  • Be available as required for some on-call duty outside of normal office hours.
  • Have a valid driver’s license, good driving record, and up to date car insurance.
