Position: CTM-FT

Shift: Over Night Shift

POSITION SUMMARY: Provide direct personal care to residents following the individualized service plan (ISP) to maximize their comfort and quality of life in a CBRF setting.

PRINCIPLE DUTIES AND RESPONSIBILITIES

1. Provide quality care and services for all residents designated in the ISP in a dignified and respectful manner to allow each resident to function at his or her highest level of independence. Includes assistance with dressing, grooming, bathing, eating, toileting, meal preparation, housekeeping, laundry, medication management and other duties to benefit the residents.

2. Provides care following the ISP and reports to the team any change in behavior or condition that may determine needed changes to the ISP.

3. Report to The Oaks Director on the current condition and needs of the resident, The Oaks Director will update the resident’s families or guardian unless it is an emergency situation.

4.Read and contribute to communication book and speak to The Oaks Director regarding any care needs of specific residents or information that is beneficial to the operations of the facility.

5. Assist other team members in providing care for residents.

6. Attend required trainings and in-service sessions to meet the required training for certification and the requirement of the facility.

7. Participate in team meetings and committees designed to improve and maintain the facility.

8. Perform all duties and responsibilities in accordance with infection control and safety guidelines as defined by facility, state, and federal regulations.

9. Become CPR certified.

6. Notify The Oaks Director or Administrator of questionable resident care or mistreatment.

7. Maintain resident, facility, professional, and personal matters confidential (including salary).

8. Is able to work outside routine working hours as necessary for the benefit of the facility.

9 Maintain a professional and neat appearance with a professional, polite, and pleasant demeanor.

10. Develop and maintain positive working relationships inside and outside of the organization.

11. Must follow cell phone use guidelines: cell phone use is only allowed for emergencies and should be used in private areas and not near residents unless authorized by The Oaks Director.

KNOWLEDGE, SKILLS, AND ABILITIES

1. Experience in the healthcare field is preferred but not required. High school diploma or GED preferred.

2. Within 90 days, must have met all state required certifications per DHS 83.20.

3. Requires ability to positively relate to the needs of the CBRF clients and have a genuine appreciation and interest in older people and those with special needs.

4. Must be able to speak, read, and write English in a proficient and understandable manner.

5. Must be able to physically perform all duties on physical profile form.

6. Must be willing and able to make independent decisions in the best interest of the organization.

7. Requires patience, tact and willingness to work with and communicate effectively with people regardless of the level at which they are currently functioning.

8. Must be able to work effectively and politely with the public under all conditions.

9. Must meet the requirements of the criminal background check law.

10. Regular and punctual attendance is expected and required.

WORK AREA AND WORKING CONDITIONS

1. Main work areas include resident rooms, dining room, kitchen, laundry area, common areas, patio, and sidewalks around the facility

2. May be subject to hostile and emotionally upset residents and family members.

3. May be exposed to injury from falls, bumps, to hot humid conditions, and may be exposed to infectious waste, disease, and hazardous chemicals.

PHYSICAL DEMANDS

1. Must meet physical demand requirements of the facility.

2. Will be able to operate various medical equipment.

3. Will be able to push, pull, move, or lift objects as listed in the physical demand requirements.

4. Will be required to walk and stand a majority of the time while performing the duties of the position.

5. Must be able to cope with mental and emotional stress related to the position and constantly changing priorities and interruptions.

6. Must possess normal sight and hearing senses or use assistive devices that will enable these senses to function adequately so that the requirements of the position are met.

7. Must be able to calmly participate in the evacuation of residents during emergency situations.
