Description: The Care Manager - RN is responsible for the coordination and delivery of care at the appropriate level and implementation of discharge planning activities for patients within the acute care setting by providing a systematic, collaborative approach in the management of patients that assures quality and cost-effective patient care, appropriate documentation to support the level of care and continuity of care after discharge.
Minimum Qualifications: Graduation from an accredited School of Nursing. Current NC nursing license or license from a compact state, three years of recent clinical experience required, previous case management experience and CCM certification required within one year.
