SUMMARY

Ensure the safety and security of the clients. Responsible to assist clients to obtain needed services by providing information, referral coordination and follow-up. Member of the clinical team working five shifts; including one evening shift and one weekend shift.

PRIMARY RESPONSIBILITIES

  • Provide program coverage, supervise daily/nightly and weekend activities.
  • Coordinate medical behavioral services through insurance.
  • Coordinate MAT and psychiatric services.
  • Make referrals for external resources including, medical, dental, health benefits, job training, Mass Rehab, career center, etc.
  • Maintain accurate client files and records, including documentation of activities.
  • Assist clients with issues of housing, education, vocational and employment as identified upon admission.
  • Facilitate in-house recovery and/or skill building groups as assigned.
  • Supervise client compliance with the program guidelines.
  • Consult with team in developing a collaborative plan of care for clients and development of aftercare plan.
  • Assist with client’s aftercare and discharge.
  • Fill in required shifts in emergency coverage situations.
  • Contribute to a supportive interpersonal atmosphere.
  • Supervise medication process and times.
  • Meet bi-weekly for supervision.
  • Must be able to drive the SMOC Program van to transport clients.
  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.
  • Attend & participate in team meetings as requested and communicate effectively with clients and staff in other areas.
  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.
  • Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.
  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Bachelors degree or 5 years relevant work experience in social services, recovery or mental health or housing services.
  • Understanding of the disease concept of addiction.
  • Knowledge of 12 Step programs.
  • Understanding of mental health diagnosis and trauma.
  • Strong verbal and written communication skills.
  • Must have valid Driver’s License, reliable transportation and meet insurance standards.

PHYSICAL REQUIREMENTS

  • Ability to drive a van.
  • Ability to climb stairs in a residential building.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Clinical Director. 3
  • Direct reports of this position are none.

WORKING CONDITIONS

Some travel within the state using a company van will be required. This position is located in a residential program which is handicapped accessible. As part of the responsibilities of this position, the Care Coordinator will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
