POSITION SUMMARY:
The WISe Care Coordinator will maintain the integrity of the Wraparound process and ensure community based care to the children and families of the WISe wraparound program. You will function as a core member of a collaborative care team. The ideal candidate will display a compassionate nature, be knowledgeable about mental health care practices, and provide exceptional service. Communication, confidentiality and professionalism are essential parts of this position. The ability to work flexible hours including evenings and participating in an on-call rotation (for which there is a pay differential) is expected.

RESPONSIBILITIES:
The primary duties and responsibilities include, but are not limited to, the following:

  • Maintain on average a caseload of 10 families.
  • Engage and assemble a child and family team, utilizing both formal and informal support systems.
  • Assess child/youth situations, and strengths to determine the services to meet their needs.
  • Coordinate and facilitate monthly Child and Family Team meetings, as well as develop an individualized care plan based on the family’s strengths, needs and their preferred solutions.
  • Develop and review individualized support plan with families.
  • Meet with child/youth and family on a weekly basis and/or as needed to partner with the family to assist in achieving their individual goals.
  • Complete all necessary paperwork and ensure case notes are completed in a timely manner in accordance with program requirements.
  • Attend all Care Coordinator meetings, individual and group coaching and training, or make prior arrangements with the Program Supervisor to be excused.
  • Maintain accurate information, ensuring assigned family’s demographic information is up-to-date.
  • Participate in the 24 on-call rotation for the program.
  • Model and encourage direct, professional communication among staff members and facilitate development of a positive staff support network.
  • Function in accordance with the LCSNW personnel policies and procedures and administrative practices of the North Puget Sound district.
  • Demonstrate respect for and build on the values, preferences, beliefs, culture, and identity, of the child/youth and family, and their community.
  • Provides coaching related to activities of the Behavior Interventionist, Youth Partner, and Family Partners as developed in the Child and Family Team. Assist Program Managers and Coaches in providing support, feedback, and problem resolution to the Direct Support, Family Partners and/or Youth Partners.
  • Working alongside families to create a plan for care after their transition from the WISe program.
  • Demonstrate and implement ethical decision making in accordance with Ethics Training provided upon hire
  • Required to travel throughout the Greater Puget Sound region. Mileage reimbursement allocated monthly.
QUALIFICATIONS:
  • Ability to read, write and speak fluently in English are required; preference for candidates who can communicate in an additional language: Somali, Dari, Farsi, Pashto, Arabic, or Spanish. Candidates with demonstrated language competency are eligible for language-based pay increase.
  • The ability to facilitate effective team meetings and group discussions.
  • Proficient in the use of personal computer and corresponding programs; preferred experience with Microsoft Office, Gmail and Google Calendar or similar web-based and computer programs.
  • Dependable, flexible, punctual, solid organizational skills,strong work ethic and service oriented.
  • Be able to respect and work cooperatively with a variety of persons, including coworkers, community partners (social service agencies), the public, volunteers and parents of the children.
  • Support and uphold the mission, beliefs and values of Lutheran Community Services.
  • Valid driver’s license, proof of Insurance and reliable transportation is required.
  • Clearance through the Washington State Patrol (background check) and verification of ability to work around vulnerable populations is required.
  • Clearance of Motor Vehicle Report to meet LCSNW insurance requirements.
EDUCATION AND/OR EXPERIENCE:
  • BA/BS with a major in social work, psychology or related field or Certified Peer Counselor with the State of Washington w/a minimum one-year experience
  • 1-2 years’ experience in a related field/program preferred
CERTIFICATES, LICENSES, REGISTRATIONS:
  • Submit Washington State Agency Affiliated Counselor Registration Application (Within 30 days)
  • CPR/First Aid (Within 90 Days)
  • Washington State CANS certification. (Within 90 Days)
