If you are an Experienced Support Coordinator that is open to working in Northern New Jersey and looking to thrive, then 7TH & Madison Extensions of Empowerment may be in your future. We are looking for experienced Support Coordinators to join our team in Northern New Jersey to serve families in Essex and neighboring areas (including Hudson, Bergen and Passaic). This position is a combination of remote work and field visits, so candidates should have the ability to travel in these areas. As a part of our team, you will have a flexible schedule, the ability to work from home on some days, and will be paid for training.

Part-time Support Coordinators require availability during the morning hours with flexibility and maintain a minimum caseload of 15 members to be built over a 6- month period or less at their manager's discretion. Total caseloads are to be determined by the Support Coordinator and Support Coordinator Supervisor and not to exceed 24 members.

Please do not apply if you do not have 1-3 years of consistent Support Coordination experience and a Support Coordination Certificate from Rutgers, Boggs Center.

Qualifications:

Bachelor's Degree with major coursework in Sociology, Social Welfare, Psychology, Gerontology, or other related social sciences and two years of experience as a Support Coordinator.

Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, work may be required on a weekend for appointment coverage, travel throughout the assigned region may occur and may be in adverse weather conditions. Must be a valid licensed driver and have a personal vehicle that is registered and insured.

Support Coordinator Responsibilities:

Adhere to all the responsibilities required by the NJ Division of Developmental Disabilities (DDD) including:

  • Explaining to members and their families the Fee-for-Service individualized budget system supporting people with developmental disabilities in NJ
  • Superb communication skills (oral and written)
  • Working with the participant and family in creating a support and service plan that details: how best to support the person, defines the needed services and their cost while ensuring the plan is within their budget, selects providers that will implement the service plan
  • Monitoring the implementation of the plan with monthly phone contact, quarterly face-to-face visits, annual home visits, and subsequent reports/meetings
  • Revising the service plan as needed to reflect the changing goals and needs of the member and or family
  • Assisting the member and family in successfully navigating the complex social service and provider system supporting individuals with disabilities in your coverage area
  • Effectively listen, communicate, and provide compassionate support to the consumer and their family
  • Learning and becoming comfortable discussing the financial and legal issues facing people with disabilities and their families including Guardianship, Wills, and Special Needs Trusts
  • Inputting and updating all support activity as required by DDD and 7TH & Madison
  • Responding quickly (within 48 hours or less) to all requests for information from: consumers, their family, service providers, DDD officials, and 7TH & Madison management regarding the members you serve
  • Proficiently and effectively use technology in communication with members and preparing service plans and DDD budgets
  • Represent 7TH & Madison in a professional manner upholding the values it represents at all times
  • Meet all required monthly deliverables

Support Coordinator State of New Jersey Requirements:

  • A Bachelor's degree in any subject
  • One year of direct experience with adults (18+) with developmental disabilities including professional, volunteer, and family experience
  • Ability to pass fingerprinting and drug screening
  • Must have your own reliable transportation as regional travel is required
  • Automobile and Valid Insurance, Valid New Jersey Driver's License, acceptable Motor Vehicle Record Check

Preferred Qualifications:

  • Previous or current experience as a Support Coordinator
  • Bilingual, including American Sign Language
  • Resident of the State of NJ
