TITLE: Clinical Pharmacist Outpatient

JOB OVERVIEW: The clinical pharmacist is responsible for providing pharmaceutical care services to clinic patients including but not limited to medication therapy management, drug therapy consultation, chronic disease statement management, participation in transitions of care, and provision of patient education and counseling. Additionally, the clinical pharmacist will participate in clinic and pharmacy staff development endeavors, participate in patient care coordination meetings and be an active participant in the clinical practice guideline review and development process.

DEPARTMENT: Hospital Outpatient Services, Pharmacy Support Systems, Specialty Pharmacy, Anticoagulation

WORK HOURS: Assigned Days and Hours; variable upon shift/setting/role

REPORTS TO: Department Manager/Supervisor or equivalent/designee

PREREQUISITES:

  • Graduate of an accredited College of Pharmacy; PharmD degree strongly preferred

  • Licensed as a Registered Pharmacist in the State of Washington with license in good standing, or eligible for licensure.

  • Completion of an ASHP accredited PGY1 residency required or equivalent practice experience. PGY2 residency preferred for specific practice areas (e.g., Oncology).

  • American Health Association's Basic Life Support (BLS) for Healthcare Providers - active certification, or required within 30 days of employment

  • Minimum of 2-3 years of clinical experience in practice setting preferred.

  • Board certification or equivalent advanced practice specialty highly desirable.

  • Essential Responsibilities and Competencies:

  • Adheres to department of pharmacy and organizational policies and procedures in all areas of practice.

  • Responsible for providing high-quality pharmaceutical care to ambulatory patients of Valley Medical Center and its Clinic Network. This includes medication reconciliation and all aspects of therapeutic selection, implementation, monitoring, modification and dispensing of drug therapy (as applicable to practice setting) - with appropriate documentation and communication.

  • Responsible for providing comprehensive patient education and counseling regarding medication therapy including use of administration devices (e.g., injection devices, inhalers, etc.) promoting safe and effective use to facilitate an appropriate therapeutic outcome for the patient.

  • Collaborates with providers and other health care professionals to individualize appropriate drug therapies based on patient parameters and interpretation of current literature, safety, efficacy, and cost.

  • Participate in medication therapy management including monitoring and modifying drug therapy upon physician referral as outlined in collaborative drug therapy agreements (CDTA), as applicable to practice setting.

  • Participate in chronic disease state monitoring and utilize registries and/or other databases as available to enhance therapeutic outcomes through disease/medication education, adherence monitoring and drug therapy optimization.

  • Participates in interdisciplinary team activities to develop and implement clinical plans of care and disease state management programs involving medication therapy, collaborative practice, treatment protocols and medication error reduction

  • Provide pharmaceutical care oversight and consultation for high-risk patients including those with 10 or more medications, high resource utilization (e.g., frequent ED/UC visits), advanced age or on high risk or specialty medications, as applicable to practice setting.

  • Participate in transition of care endeavors including collaborating with inpatient providers for complex patients (e.g., CHF) and perform medication reconciliation for hospital discharge patients, as applicable to practice setting.

  • Identify patient barriers to medication adherence taking into account patient characteristics (including language, cultural/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status, etc.)

  • Responsible for documenting drug information questions, drug utilization evaluation activities and adverse drug reactions.

  • Participates in the development and documentation of quality assurance indicators such as drug use evaluation.

  • Maintain clinical competency through successful completion of the self-study modules and providing clinical services in practice.

  • Provide teaching, direction and supervision of pharmacy residents, technicians, interns and student, as well as training of new pharmacists.

  • Participates in projects as determined by the strategic goals and initiatives of the department and organization.

  • Assists in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options and promotes the adherence to the VMC Drug Formulary as approved

  • Conduct periodic reviews of prescribing practices and design targeted prescriber education to promote compliance with clinical practice guidelines

  • Actively participate on committees responsible for medication use, quality improvement, performance improvement, and other aspects of patient care within the health system

  • Maintain knowledge of outpatient medication reimbursement models including MTM and pharmacist provider status.

  • Develops and presents periodic in-services to healthcare providers.

  • Participates in research studies in the clinical service.

  • Maintain competency in all applicable CDTAs and completes all department and organizational competency assessments as assigned.

  • Completes additional responsibilities/projects as assigned by Supervisor/Manager/Director.
