Essential Duties

The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.

• Coordinate services and supports, provided by multiple agencies and synchronized to address the needs and strengths of each child or youth following strength-based child and family team approach to develop a service plan.
• Ensure case management strategies focus on collaborative practice and include implementation of quality visitation and contact, evidence-based direct therapeutic services to children and supportive services to parents, coordinated case plans and effective communication protocols across child welfare, treatment providers, and court systems.
• Monitor child victims and their families and track service linkage.
• Track all direct service data for each child for a six month period. Provide follow up support services and communication during second six months.
• Review program service agreement that outlines the goals and objectives of the program with each family and that requests a commitment for active participation.
• Ensure service providers incorporate a trauma-informed perspective their practices to enhance the quality of care for these children.
• Ensure resources on trauma are available to providers, survivors, and their families and that there is a continuity of care across service systems.
• Facilitate all trainings using the Resilience and Paper Tigers and track all data.
• Other duties as assigned.

Minimum Qualifications

Requires a Bachelor’s Degree in Social Work from an accredited school of social work; or a Bachelor’s Degree in a human service field with at least 25 semester hours of approved coursework and one year of experience in case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning; or a four year degree and two years of related experience. A copy of a transcript must be included with the application. The applicant selected must undergo a criminal background check and pass a drug screening test prior to employment

Pay: $20.23 - $31.35 per hour
