Landmark Home Health is looking for Registered Nurses in Monessen, Belle Vernon, Perryopolis, Fayette City, Clairton, Charleroi and surrounding areas with strong clinical skills, computer skills, and of course Home Health Care experience is not required but a PLUS!

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Initial evaluation and assessment of patient needs.
• Establishes a nursing care plan, implements this plan, and evaluates patients progress. Sets goals with patient, patient's family and physician.
• Maintain safe environment for patient.
• At regular intervals, reports patient condition to physician.
• Completes verbal orders and maintains an up-to-date medical record.
• Provides services requiring substantial specialized nursing skill.
• Communicates, on a regular basis, with other disciplines involved in the care of the patient.
• Promotes optimum health levels for patients.
• Attends regularly scheduled staff meetings.

QUALIFICATIONS:

• Current Pennsylvania Nursing License is required
• Must have one (1) year of current nursing experience
• Valid current driver's license and auto insurance coverage is required
