This position will work a hybrid schedule which includes a combination of working both onsite at Greenwich Drive in University City and remote.

DESCRIPTION

The Digital Health (DH) RN Case Manager (CM) is responsible for monitoring the daily remote patient monitoring dashboard and calling patients whose readings are not within range (Blood Pressure and Blood Sugar only at this time but will expand to other disease states). The DH RN CM handles any incoming calls and questions by patients and can provide heath coaching and education, escalation to providers, and community resources linkages. Other duties as assigned.

MINIMUM QUALIFICATIONS

  • Must be a graduate from an accredited school of Nursing.

  • Registered Nurse (RN) license issued by the state of California.

  • Prior case management, utilization review, and discharge planning experience. If insufficient applicant pool, department is willing to consider candidates without case management experience who have three (3+) or more years of recent clinical RN experience including triage experience.

  • Strong teamwork and interpersonal skills.

  • Must have proven autonomous problem solving, independent thinking, decision making, and prioritizing skills.

  • Proficiency in Information Systems and computer programs including MS Office (Word, Excel, Outlook, PowerPoint, etc.).

PREFERRED QUALIFICATIONS

  • BSN/MSN

  • CCM

  • Three (3+) or more years of clinical RN experience including triage.

  • Proven outpatient, managed care, or similar case management experience.

SPECIAL CONDITIONS

  • Must be able to work various hours and locations based on business needs.

  • Employment is subject to a criminal background check and pre-employment physical.

  • This position is a hybrid of up to three days on site and two days at home. On-site work required during the training period, then hybrid work from home and office when skills are proficient.

Pay Transparency Act

Annual Full Pay Range: $136,404 - $179,927 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $65.33 - $86.17

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
