Job description

Seeking a motivated nurse for a Home Health and Hospice position in Newport and surrounding areas.

Job duties include:

• Be a Registered Professional Nurse currently licensed in the state in which employed by agency, with at least one (1) year experience in nursing.
• Conducts an initial evaluation and completes the hospice and/or home health admission process for all appropriate patients.
• Obtains a medical history, particularly as it relates to the present condition, from the patient and/or family member.
• Conducts a physical assessment of the patient.
• Must be able to lift 50 lbs. or greater.
• Evaluates the patient, family member(s) and home situation to determine what health teaching will be required.
• Evaluates the patient's condition and home situation, determines if the services of other disciplines are needed and communicates need to Physician.
• Provides treatments and procedures requiring substantial specialized skill in accordance with the Physician's order.
• Observes signs, symptoms and reports to the Physician reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teaches, supervises and counsels the patient and family member(s) regarding the nursing care needs and other related problems of the patient at home.
• Supervises and evaluates the care given by the Home Health Aide as needed.
