Job Description
Job Purpose:
Under the supervision of the Chief Operations Officer, clinic physicians, and nurse practitioners, the RN functions in a team and as a mission-based clinic nurse in an outpatient primary health care clinic serving the low income, working poor, and homeless. Utilize the nursing process in providing patient care, case management, and education focused on treatment compliance and health risk factor reduction. Perform diagnostic laboratory tests, therapeutic procedures, and patient education under the direction of the clinic physician. Ensure the efficient and effective flow of patient during scheduled clinics through joint planning and problem solving with clinic staff and volunteers. Protect the dignity, privacy, and confidentiality of patients and families, as well as so-workers and others. An important component of this job would also be to serve as a “liaison” between the medical providers and Chief Medical Officer (CMO). Relationship building among staff and providers will be necessary for the success of this job duty.
Principal Duties and Responsibilities:
  • Adhere to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties.

  • Oversees the care essential to the assessment, promotion, maintenance and restoration of patients’ health and wellness.
  • Oversees patient needs and suggests solutions to patient care problems.
  • Acts as a liaison between medical providers and CMO. In performing this duty, the employee will serve as an initial line of communication regarding medical provider issues. These issues will then be triaged and taken to appropriate leadership for resolution.
  • Ensures compliance with regulations and with clinic standards of quality patient care.
  • Interprets to the patient and family diagnosis and treatment plans consistent with individual skills and the wishes of the clinician.
  • Communicates information or results to patient as directed by the medical provider/clinician.
  • Teaches basic principles of healthful living and health risk factor reduction.
  • Responds to phone calls from patients, triage requests and routing them to the appropriate staff or community resources.
  • Maintains a good working report with Clinic Manager to ensure smooth ongoing medical functions.
  • Oversees Employee Health process and coordinates with HR for maintaining compliance throughout the clinic system.
  • Maintains accurate and legible records of interventions.
  • Performs related duties necessary to ensure quality patient care as requested by supervisor.
  • Responsible for ensuring and directing medical equipment maintenance when needed.
  • Responsible for oversight of immunization program, assessing client needs, administration, recording, and assuring vaccines are maintained according to clinic and state/federal policies and procedures.
  • Coordinates the storage and ordering of on-site medications.
  • Family Planning: Able to instruct client in all forms of family planning methods, dispense birth control methods per family planning pharmacy protocol.
  • Performs any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.
Licensure/Certification Requirements:
  • Licensed in Oregon; Fully Credentialed and Insurable
  • RN from an accredited school of Nursing, BSN preferred
  • Current Basic Life Support Card
  • Non-violent Crisis Intervention Comprehensive training class completed within 6 months of hire and recertify prior to the expiration date.
  • To obtain CPR Instructor Certification within 1 year of the hire date.

  • 0 – 60 months of service = 144 hours per year/18 days
  • 61 – 120 months of service = 200 hours per year/25 days
  • 121+ months of service or more = 240 hours per year/30 days

Semi-Annual Retention bonus
  • Opportunity to earn up to 2% of your annual income twice per year as a bonus. One bonus will be given in November and the other in May. This will be (3) yes or no questions based on goals you make with your supervisor. There is an individual goal, a department goal, and a company goal.
