Job Summary

The Registered Dietitian is responsible for the provision of high quality nutrition care to the patient. Monitors and assesses the nutritional status of patients as it relates to disease states and clinical conditions. Develops and implements nutrition care plans. Counsels and instructs patients according to prescribed diets. Communicates nutritional needs and concerns with the health care team and documents these results in the medical records. Coordinates clinical nutrition activities for best patient outcomes, in collaboration with the Clinical Nutrition Manager.

Essential Functions

  • Serves as the liaison between Clinical Nutrition, Food Service and Nursing Units; communicates information, and participates in productive problem solving.
  • Oversees the nutrition screening process to identify nutritional risks and potential problems
  • Follows department standards of care in the assessment of patient nutritional needs
  • Develops, implements and continuously evaluates plans of care by setting realistic obtainable goals, including the patient in the process and by following department timeliness criteria and standards.
  • Communicates nutritional concerns or problems to other members of health care team through multidisciplinary patient care rounds and routine interactions on the unit.
  • Maintains accurate records of each patient contact and follow-up, including clinical monitoring forms and diet office records of food preferences and allergies, food-drug considerations, and other pertinent dietary information.
  • Designs appropriate and accurate meal plans for the diet clerk to follow when complex modified diets are ordered, making adjustments in foods served based on diet guidelines and/or patient preferences.
  • Participates in decision-making process for problems related to patient nutritional care and the food production process.
  • Responsible for patient Medical Nutrition Therapy education, including provision of follow-up resources
  • Provides nutrition education as ordered by the physician, verbally clarifying any discrepancies or inconsistencies between the assessment and the MD order.
  • Performs discharge diet instructions and consults/referrals for education within 24 hours of physician order.
  • Serves on department and hospital committees and work groups as assigned
  • Participate in the development and revision of department policies and procedures.
  • Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. C-I-CARE is the foundation of Stanford Healthcare – ValleyCare’s patient-experience and represents a framework for patient-centered interactions

Job Qualifications

Education

  • Bachelor’s degree or advanced degree from an accredited college or university in Food Science, Nutrition, or Dietetics.

Experience

  • One-year experience in clinical dietetics, preferred

Licenses and Certifications

  • Active Registered Dietitian (RD) status, or registration eligible and must sit for the examination with 3 months after completing eligibility requirements.
  • Maintains all requirements established by the Commission on Dietetic Registration.
  • Certified Dietitian Educator (CDE), Preferred

Knowledge, Skills, and Abilities

  • Strong organizational skills and the ability to accomplish assigned work with time constraints
  • Good written and verbal communication skills
  • Strong command of the English language
  • Strong interpersonal skills, ability to work productively with people from all backgrounds
  • Ability to be a team player, but also able to work autonomous
  • Knowledge of hospital Joint Commissions, infection control safety and security guidelines
  • Ability to work varying shifts as needed (nights, weekends, holidays)
