Description:
Jefferson County Public Health is seeking to hire a Family Connects Public Health Nurse to work on the Maternal Child Health team. The Family Connects Public Health Nurse provides home visits to all families with newborns in the identified catchment area. The goal is to assess family needs in the twelve domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The home visiting nurse will work in a team environment of clinical and non-clinical staff to schedule, visit, and connect families to community resources using the evidence-based Family Connects home visiting protocol. Ability to communicate effectively with others is essential. Experience in maternal and child health, home visiting, lactation education and fluency in Spanish is desired.

This position will be expected to establish and maintain relationships and facilitate collaboration among community partners, and other JCPH and county programs to implement evidence-based strategies, leverage community actions or resources, and monitor progress to ensure sustainability of the project efforts. With significant autonomy, this position tailors the plan for care based on the needs of the family, implementing, coordinating, and monitoring progress. Performs all evaluation processes, including documentation and reporting.

Background on the Family Connects program:
The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally.

Schedule: This full-time position has the potential to operate on our four-day work week (Monday-Thursday), or five-day work week (Monday-Friday). There is flexibility with scheduled work times (start and/or end times). This position is hybrid, with a large portion of the nurse’s work occurring in the community conducting home visits in addition to remote and in-office work as required.

Essential Duties:
Deliver individualized, culturally responsive, ethical, person-centered care to individuals and families utilizing reflective communication skills.

Adhere to the Family Connects home-visiting protocol. Maintain records of client observations and activities as required. Document information for family assessments and program evaluation per program guidelines.

Adhere to the Family Connects scheduling protocol. Visit target area hospitals as needed to schedule home visits for new mothers.

Provide a health and physical assessment of birth parent and newborn in the home to support optimal health and well-being.

Assess family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence. Exercise clinical nurse judgement skills in appraising situations and making decisions.

Establish a positive rapport with families during in-home visits and co-develop a family support plan with goals, objectives, and activities that address family strengths and needs.

Work with Family Connects team to ensure appropriate referrals and connections to community resources as needed. Act as a liaison between families and other community agencies, serving as family advocate when needed.

Apply working knowledge of parent-child interaction, maternal and child health, child development, and trauma-informed care.

Respond to client needs through flexible scheduling of home visits, including occasional home visits during evening or weekend hours.

Establish and nurture relationships, partnerships, and coalitions with community resources and services that provide support to families and participate in the community system of care for young children and their families.

Work collaboratively with the Family Connects team to identify resources, establish relationships with providers, maintain an updated list of resources, and identify gaps in needed community resources.

Attend and actively participate in case conferences, regular team meetings, clinical supervision, and other meetings as required.

Qualifications:

Minimal Qualifications:
Bachelor’s Degree in Nursing and one year of nursing experience

OR

Associate’s Degree in Nursing and three years of nursing experience

Licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact at time of employment.

Current CPR certification at hire

Preferred Knowledge, Skills and Abilities:
Experience in perinatal nursing

Experience in public or community health nursing

Bilingual in Spanish/English highly preferred

Excellent interpersonal and relationship-building skills to work collaboratively with families, co-workers, community members and organizational partners

Strong strategic and critical thinking skills

Ability to manage competing demands, adapt to rapid schedule changes and work independently to meet expected timelines

Ability to accept personal differences, to establish trusting relationships, and to work well with diverse populations through inclusive and culturally responsive practice

Must provide immunization record at time of hire or prove immunity to comply with departmental Communicable Disease Control Policy within 30 days of hire date.
