General Description:
The Family Enrollment Specialist/Program Supervisor is responsible for implementing the Healthy Families Program as outlined in the HFA and HFAZ policies and procedures; directly supervise the Family Support Specialists and performs enrollment duties. May carry a caseload of up to 6 families. This position reports directly to the Program Director.

Summary of Essential Job Functions:

  • Supervisor conducts the following when assigned:
  • Coordinates supervise and participate in all activities of the Healthy Families program.
  • Community outreach and partnership building.
  • Program presentations.
  • Provides strength-based, reflective supervision with each staff member weekly.
  • Maintains and monitors staff caseloads and work assignments.
  • Ensures that necessary data is collected and provided to program data entry staff in a timely manner.
  • Coordinates program activities with hospitals, Child Protective Services, and community-based agencies.
  • Attends and participates in all required Healthy Families seminars, meetings, and in-service training.
  • Performs other related duties as required and or assigned.
  • FES Duties are as follows:
  • Conduct in-hospital or clinic outreach or screening duties as outlined in the MOU.
  • Establish and sustain partnerships within the community.
  • Attend identified community coalition and service group meetings as assigned.
  • Receive and process daily, referrals from hospital personnel as well as other agencies.
  • Maintain a referral organizational system.
  • Schedule assessments meeting the needs of the family.
  • Adhere to HFA standards regarding contacts, time frames, assessment rates, etc.
  • Complete quarterly FES reports.
  • Offer and administer the FROG virtually or in person.
  • Ensure referral to the Family Support Specialist for continued follow-up and support services.
  • Creates electronic family files, and uploads to FSS’s folders.
  • Maintain client records in accordance with program and funding source requirements.
  • Meet weekly with Family Assessment Supervisor to discuss referrals and narratives.
  • Attends parent events.
  • Make DCS referrals as appropriate.
  • Provide community presentations and community outreach activities.
  • Maintain professional positive relationships with hospital staff and other collaborative agencies.
  • Will assist the program as assigned/needed when enrollment openings are limited.
  • Perform other related duties as assigned, especially when no enrollments are needed.

Requirements:

  • Bachelor’s degree plus two years of experience working with at-risk families, and one year of supervisory experience preferred.
  • Ability to mentor, supervise and support staff in person or virtually.
  • Knowledge of family life skills, and child abuse prevention techniques.
  • Flexibility in scheduling (able to work limited nights and weekends).
  • Ability to work effectively within a team.
  • Knowledge of community resources and existing community prevention efforts.
  • Ability to communicate effectively both orally and in writing.
  • Demonstrates effective problem-solving skills.
  • Grant writing abilities preferred.
  • Meets department performance/productivity standards.
  • Must be eligible and able to obtain a Level one fingerprint card.
  • Must be certified in CPR/First Aid.
  • Must have a valid driver's license, and vehicle and meet agency liability insurance requirements.
  • Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage, and dress code.
  • Attend required agency and program/department meetings.
  • Follow and model CFR’s core values.
  • Participate in PQI initiatives.

The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice and the position requires quick response time and flexibility.
