The Program Manager, Homelessness System Perinatal Services is accountable directly to the Vice President of Health Services. The candidate will lead the program to ensure effective use of resources, staff, partnerships, data and other networks to develop a safety net to connect people who are experiencing homelessness and are pregnant to perinatal care. This candidate will develop strong partnerships with Department of Human Services (DHC), Virginia Williams Resource Center (VWRC), staff, perinatal service providers, housing agencies, and others. This candidate will educate partners,clinicians and housing system staff on issues of perinatal health to support families during pregnancy and infancy. This position will be hybrid and include some work at Virginia Williams Resource Center as well as COH sites. This is a full-time position.
Highlighted Duties and Responsibilities:
  • Develops and manages partnerships with DHS and VWRC staff to coordinate processes and integration for handoffs, data collection, coordination, and education.
  • Coordinates closely with COH housing staff and other housing agencies and advocates to ensure appropriate care and effective collaboration
  • Supports staff in process improvements to develop the most effective methods of handoff and coordination for target population.
  • Develops educational sessions and materials to ensure that housing staff are aware of importance of perinatal health care, with an emphasis on increasing understanding of healthy pregnancy and warning signs, nutrition, self-management of care and primary care
  • Develops educational sessions and materials to ensure that health care staff are aware of barriers to receiving perinatal care for people experiencing homelessness and best practices for overcoming these barriers
  • Develops and monitors use of assessments to evaluate risks factors and connection or barriers to receiving perinatal services, preferences for care, potential risk factors, and understanding of and preparation for birth and infancy.
  • Trains and supports staff in client-centered techniques for providing information to clients on the importance of perinatal health care, with an emphasis on increasing understanding of healthy pregnancy and warning signs, nutrition, self-management of care and primary care.
  • Maintains a clearinghouse of appropriate and preferred community resources for perinatal care, in coordination with the housing case managers
  • Develops systems to measure follow up with clients or health care providers (with client permission) to ensure successful referral and attendance at appointments.
  • Reviews data in Quickbase, EClinicalWorks, CRISP and other sources to ensure that staff are adequately capturing activities for care coordination and program evaluation.
  • Attends Maternal and Child Health department and Housing monthly meetings at Community of Hope and all-staff meetings as requested
  • Participates in Virginia Williams Resource Center staff huddles (currently every other Friday)
  • Meets with supervisees regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed
  • Reviews the performance of team members in a timely manner, including completing 90 day and annual performance evaluations per their due dates, documenting any issues or changes, providing recommendations for professional development and/or training opportunities, and recognizing staff when appropriate
  • If applicable, actively recruits for vacancies with a sense of urgency. This includes reviewing resumes, phone screening, interviewing, and completing references. This also includes partnering with Talent Management for assistance in an effort to fill vacancies promptly.
  • Completes other duties as assigned by supervisor
Minimum Qualifications:
  • LGSW, LICSW, or equivalent experience required
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date
  • Experience providing social services, health care coordination or health care education required.
  • Familiarity with perinatal services and standard of care required
  • Strong written and verbal communication skills.
  • Strong organizational skills
  • Experience working with under resourced population, including pregnant people experiencing homelessness, preferred.
  • Familiarity with Medical Assistance system and community resources strongly preferred
  • Ability to work on site and to travel between sites required
  • This position is considered Hybrid – remote work and on-site work requirements are split roughly 60/40. Please note that the remote work agreement is subject to change at any time, and the supervisor may require in person learning for a specific amount of time after hire
  • All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines. Medical or religious exemptions may be requested.
