GENERAL SUMMARY:

The Practice Transformation Specialist works with assigned practices and organizational teams through dedicated practice support services to ensure successful implementation of workflows, procedures and project goal accomplishment to consistently achieve the Triple Aim goals of improved health outcomes, improved experience of care and improved per capita cost of care for covered individuals.

Qualifications - External

EDUCATION/EXPERIENCE:

  • Bachelor's Degree in a health-related field is required.
  • Two years of experience in clinical quality, practice transformation or program management support required.
  • Experience providing practice transformation services for clinical network, provider organization or related entity preferred.
  • Demonstrated effective verbal and written communications, and interpersonal skills necessary to interact and build strong relationships with physicians, management, staff, external agencies, customers and patients/families.
  • Proficient computer skills in word processing and spreadsheet software.
  • Ability to interpret performance data from multiple sources to determine and implement effective action and solutions.
  • Demonstrated capability to prioritize multiple conflicting duties, frequent travel and an ability to adapt to diverse customer population.
  • Experience and/or ability to demonstrate comprehension and ability to effectively communicate medical terminology and medical disease processes.
  • Reports to the Director of Payor Relations – Population Health. Responsibilities will include monitoring, evaluating, and reporting quality and utilization performance on various payor contracts; working with ambulatory clinicians to identify gaps and develop action plans for improvement; and support process improvement efforts. Candidates should possess strong analytical, process improvement, and communication skills – both written and verbal.

Job Type: Full-time

Pay: $53,622.00 - $75,088.00 per year
