St. Catherine of Siena Nursing & Rehabilitation Care Center has been serving Smithtown and its surrounding communities since 2000. We are a 240- bed, hospital-based, skilled nursing facility providing sub-acute, short-term rehabilitation and long-term care. St. Catherine of Siena provides quality, patient-centered care and is a proud member of Catholic Health.

Our expanded rehabilitation department is affiliated with St. Charles Hospital Rehabilitation and is open seven days a week. The program is staffed with licensed, professional specialists in physical, occupational and speech-language pathology. We offer quality, state-of-the-art rehabilitation programs and modalities, to ensure patients' transition into their homes and communities at their optimal level of physical functioning. In addition, specialized programs in cardiac recovery, stroke rehabilitation, wound care, pain management, amputee care and orthopedics are offered, to better assist patients with a wide variety of health management challenges.

DUTIES:

• Coordinates with the Medical Director, Attending Physicians, Nursing Staff, Rehabilitation Staff and Ancillary Support Staff in providing the direct and collaborative treatment plan for sub-acute, rehabilitation and long-term care residents.
• In collaboration with the Medical Director and Attending Physicians, performs physical examination, comprehensive patient assessment, medical diagnosis and prescribes appropriate treatment modalities for this resident population.
• Ensures that all tests, therapies, medications and treatments are initiated, followed and evaluated timely.
• In collaboration with the Director of Nursing, monitors that the Nursing Home's policies & procedures and nursing standards of care are appropriately followed and maintained. Reviews/audits nursing assessment, planning, intervention, evaluation and medical record documentation for regulatory compliance.
• Provides resident, family and staff education and participates in Performance Improvement.
• Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights.

QUALIFICATIONS:

• Graduate of an accredited school of Physician Assistant Registered Professional Bachelor of Science, Masters preferred in an AMA approved PA or NP program
• NYS Physician Assistant or Nurse Practitioner License
• PA or NP Board Certification
• BLS and ACLS certification
• Prior experience in acute, sub-acute, rehabilitation and long term care preferred.
