At LINK Home Therapy we’re looking to hire a passionate Physical Therapistinterested in working in independent living and assisted livingsetting as a PRN employee.Mentoringtraining, simple documentation, great communication andan amazing work culture all contribute to a working experience that you’ll look forward to when the alarm clock rings
Benefits:
-Rate Range: $55-60/hr
-Flexible Schedule

Qualifications:
-Graduated from an accredited Physical Therapy program or university
-Valid professional license and registration for state of WI
-COVID vaccination mandatory per site requirements 

Therapist Responsibilties:
-Assist with retrieving scripts/doctor’s orders as requested by DOR
-Complete evaluations, daily notes, weekly progress notes, recertifications, and discharge documentation in a timely manner and in accordance to CMS guidelines
-Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician
-Supervise assistants as requested by DOR
-Collaborates with all disciplines to provide best patient centered treatment and care
-Adhere to scheduled hours and demonstrate professionalism at all times
