Department: Case Management
Work Hours: Part Time (Two 12 Hour shifts per week)
12 hour shifts 7a-7p and 10a-10p weekdays with weekend and holiday rotation
The Patient Flow Supervisor (PFS) screens, reviews and facilitates the inpatient admissions, observations in the Emergency Room and direct admissions thru Registration. Facilitates accurate documentation for severity of illness and medical necessity. Must function independently with excellent attention to detail, be able to multi-task and receive information from multiple sources while continually establishing priorities. Works collaboratively with physicians, case managers, and nursing staff, and provides guidance and recommendations for inpatient admission, observation status or discharge. Responsible for transition of care from the ED to include assessment, consultation, education, and referrals to various community agencies and resources.
Requirements
Graduate of an accredited bachelors in nursing program, current active nursing license in Pennsylvania and a minimum of five years acute care experience in the Emergency Room, Critical Care, PACU or Adult Med/Surg. Knowledge of Pathophysiology and Disease Process. Knowledge of regulatory environment. Must possess critical thinking, problem solving and excellent interpersonal skills. Must be able to effectively and appropriately communicate with physicians & other clinicians. Advanced IT skills required; familiarity with Windows based software programs. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 within 90 days of commencing employment.
Preferred: Master’s degree in Nursing, Previous experience in Case Management, Billing, and Coding practices. Knowledge of Interqual, Knowledge of Medicare Part A and Part B. Knowledge of Core Measures and Patient Safety Indicators.
