Foltsbrook Center for Rehabilitation & Nursing is looking for Per Diem Physical Therapists to join our subacute rehabilitation, and skilled nursing facility. The ideal candidate must be able to demonstrate excellent patient service delivery including evaluation and treatment of routine patients. In addition to excellent patient service, we would like a candidate who is steadfast in completing all necessary documentation regarding our patients, and effectively communicates patient information across the continuum of care. A physical therapy candidate who will participate in continual training, and program development, who will represent the department properly, and effectively to the community.

RESPONSIBILITIES:

• Restore, maintain, and prevent decline in resident function by planning and administering physical therapy plans of treatment.
• Provide quality care by assessing and interpreting evaluations and test results.
• Determine physical therapy treatment plans through evaluation and consultation with the interdisciplinary team.
• Assist residents to reach goals through appropriate use of therapeutic exercises, activities, modalities, and equipment as necessary.
• Routinely assess resident progress and modify treatment plans as necessary.
• Evaluate and recommend the use and fit orthotic devices.
• Complete all documentation and billing in a timely and accurate manner.
• Instruct residents on proper in-home exercises and therapies; complete home evaluations as necessary to facilitate safe discharge planning.
• Educate residents and their families about the plan of treatment, progress, and potential outcomes.
• Supervise Physical Therapy Assistants as necessary.

Education and Experience Requirements:

• NYS Licensed Physical Therapist
• Minimum of one year of physical therapy experience in a geriatric setting preferred, but not required.
