Job Summary:

The Options Care Management Specialist provides consumers and their family-caregivers with high-quality professional practices such as person-centered assessment and service planning, care coordination, referral, resources counseling, and ongoing monitoring and evaluation of services and supports required for a safe and healthy community living, which is manifested through independence and self-determination. The development of trusting, supportive relationships with those served is critical, and it is achieved through home visits and regular check-ins, reassurance calls, resource counseling, and effective engagement of both consumers and family caregivers. The incumbent OCM Specialist works and adheres to all standing care management guidelines, policies, procedures and quality assurance standards.

Requirements:

Education:

Bachelor’s Degree in social work, occupational therapy, psychology, or related social sciences concentration – other degrees considered as applicable.

Experience

Two (2) or more years of direct experience with care/services coordination and/or resource counseling/navigation.

Social service experience with older adults, their families/partners and service delivery practices to promote maximum self-determination, functioning, and quality of life.

Collaborative transdisciplinary teamwork (e.g., teams composed of different members and professions cooperating across disciplines or systems).

Licenses, Certifications & Clearances

Act 33/34 (PA State Criminal and Child Abuse) Clearances

FBI Clearance (prepared by DHS)

Valid PA driver’s license, reliable transportation, and the ability/willingness for local travel

Knowledge, Skills, Abilities

· A passion for working with aging individual with/without disabilities and providing person-center care.

· Excellent interpersonal and communication skills.

· Ability to communicate with a range of diverse people/professionals using a variety of communication methods.

· Excellent written and oral skills.

· Ability to work under pressure.

· Optimal time management with the capacity to prioritize their own and/or others workload.

· Working/using Microsoft Office Suite, using/navigating search engines, and familiarity with SAMs or in using database-systems.

Job Duties:

· Conduct/facilitate or direct person-centered functional and/or social determinant of health (SDH) needs assessments.

· Conduct/facilitate or direct the development, implementation and monitoring of care/service goals or plans.

· Assess, monitor, track, and report on the progress, experience and well-being of seniors and their living contexts.

· Complete all substantive assignments/tasks within the scope of their OCP responsibilities (include compliance with extant performance Matrix and record- keeping policies and procedures).

· Keep up-to-date and well-informed about the policies, practices, services, and of the health, functional and social strengths/limitations involved in caregiving situations with older adults and their informal caregivers.

· Apply problem-solving strategies (e.g., analyzes problems/issues of diverse scope/complexities and formulate strategies for their solutions).

· Effectively promote, facilitate, and empowers the engagement of seniors and their caregivers in all aspects of care/helping process.

Full Time status also includes:
