MINIMUM REQUIREMENTS:

EDUCATION:

Master’s degree in social work from an accredited university.

CERTIFICATION/LICENSURE:

Fully licensed Master of Social Work (LMSW).

EXPERIENCE:

Minimum of one years’ experience providing services to individuals who severe and persistent mental illness. Minimum of one years’ experience providing services to individuals who are diagnosed with an intellectual/developmental disability.

Minimum of one year experience working with the geriatric population and/or in a health care setting preferred.

SUMMARY OF RESPONSIBILITY:

The Omnibus Budget Reconciliation Act (OBRA) Coordinator oversees all functions of the OBRA PASRR process for Shiawassee County for persons seeking admission to a nursing facility. The OBRA Coordinator is responsible for the initial screening then the coordination of all involved services to complete a full screening if deemed appropriate. The OBRA coordinator will collaborate with additional professionals such as the hospital, nursing facilities, nurses, psychiatrist, and occupational therapist to ensure accurate and timely assessment. The OBRA Coordinator is directly supervised by the Supervisor of Adult MI Services or to other supervisory staff as designated by the Chief Executive Officer.

Pay: $21.63 - $27.69 per hour
