Position Title: Medical Coordinator
Reporting Relationship: National Director of Ancillary Services

Position Summary
The Medical Coordinator for NYAP’s La Jornada (The Journey) Program focusing on Long-Term Foster Care (LTFC) will coordinate medical services and record keeping for youth who have fled their country of origin and arrived in the U.S. without a parent or legal guardian.

RESPONSIBILITIES
The Medical Coordinator will perform duties including, but not limited to:
Performing all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies

  • Responsible for following the assigned intake, placement, and discharge procedures including conducting/coordinating/reviewing/updating assigned assessments, service plans, forms, and packets and collecting and uploading assigned documents into NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.
  • Writing required and additional progress notes for assigned responsibilities, and uploading progress notes and accompanying documents to NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.
  • Coordinating the provision of medical intake assessments for each person served within 48 hours of arrival in compliance with ORR Policies.
  • Medication management and maintaining control and audit of all medical supplies including prescriptions to be administered to minors at the site.
  • Maintaining up to date medical files for minors.
  • Providing Foster Parents with clear explanation of any treatment required by minors and reporting any disease/disease requirements to the Foster Parents.
  • Providing Foster Parents with injury prevention strategies for the minors.
  • Responsible for HIPPA protection in medical information regarding the minors.
  • Ensuring all needs for medical treatment for illnesses and/or injuries and immunizations, are met.
  • Working in collaboration with Case Managers to schedule and coordinate examinations by the consulting physician for each minor.
  • Assuring the administration and implementation of physician orders, and when appropriate, educating the appropriate staff on administering medication and/or treatment.
  • Assuring all documentation and required record keeping is completed in a timely manner.
  • Preparing Treatment Authorization Request (TAR) prior to appointments and follow up after approval to coordinate service with Healthcare provider.
  • Assuring the program’s compliance with Medical Policies and Procedures as well as Standing Orders.
  • Understanding health and safety standards for out‐of‐ home child care, child care licensing requirements, and community health and mental health resources for children. Coordinating with health professions for provision of services to ensure health and safety, licensing requirements, and health and mental health needs are met.
  • Maintaining Red Cross CPR/First Aid Trainer’s Certification.
  • Performing other duties as requested.

MINIMUM QUALIFICATIONS

  • Must possess a Bachelor’s degree in behavioral science, human services, social service fields or business administration or management.
  • Experience in Pediatrics medical services coordination.
  • Understanding of Medicare reimbursement codes and Treatment Authorization Request (TAR) creation and submission.
  • Must be able to maintain compliance with attending agency required training programs.
  • Bilingual English/Spanish fluency required.
  • Proficient use of desktop and laptop computers, Microsoft Office Suite including Word and Excel, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
  • Must be able to organize, navigate, and physically escort minors to various travel destinations locally and possibly including airlines, trains, and buses.
  • A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served.
  • Must be able to embrace working as a diverse team both effectively and respectfully with persons of any background, race, color, religion, sexual orientation, ability, national origin, or culture.
  • Provide excellent internal and external customer service focused on caring for people, connecting communities, and promoting peace.
  • Must possess good verbal, listening, and written communication skills.
  • Must have a valid driver’s license, reliable transportation, automotive insurance and a good driving record.
  • Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability.
