This position is delegated to and accepts authority, responsibility and accountability for timely completion and coordination of the RAI process, time frames, reports and scores of this facility.

Principal Duties and Responsibilities

  • Assure Resident safety at all times.
  • Willing to address each Resident with his/her preferred name as well as learn and abide by Residents’ Rights, rules and regulations.
  • Commitment to work well with other staff avoiding petty conflicts that disrupt a productive work environment. Does not participate in facility gossip and rumors.
  • Knowledge in RN/LPN duties and responsibilities.
  • Knowledge of medical terminology.
  • Chair a team, possesses leadership and supervisory skills.
  • Effectively deal with geriatric population.
  • Accurately record daily documentation on electronic system.
  • Complete and submit necessary forms in accordance with state and federal regulations and facility policy.
  • Effective organizational skills.
  • Maintain confidentiality of all activities, persons, and records.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to Long-Term Care.
  • Make independent decisions when circumstances warrant such action.
  • Complete all Residents’ Minimum Data Sets accurately in accordance with federal guidelines and time frames.
  • Ensure all deadlines are met with regard to MDS assessments, completion, data entry, lock dates and transmission to state and/or federal offices.
  • Ensure that Resource Utilization Group (RUG) scores are accurately created in a timely manner and that all MDS tracking for billing logs and other necessary reports are current and submitted to the consulting firm as needed.
  • Evaluate all Medicare A Residents on a continuous basis throughout their qualifying period to ensure their care and functional needs are known and communicated and the MDS and RUG documents reflect these needs.
  • Remain knowledgeable of latest state and federal regulations regarding Medicare reimbursement in Long-Term Care.
  • Monitor Medicare charting to ensure compliance with guidelines.
  • Keep nursing and rehab staff informed of status of Medicare Residents with regard to level of care.
  • Train and prompt nursing assistants to record and keep the nurses informed of Residents’ ADL functioning levels and changes.
  • Coordinate, with management team, the receipts of hospital documents needed to accurately reflect RUG scores.
  • Refer complex referred Resident information to consulting firm, as appropriate, for pre-admission estimation of per diem costs.
  • Keep facility Director of Nursing informed of problems and Medicare-related issues.
  • Conduct regularly scheduled and ad hoc Medicare team/committee meetings to review skilled residents for progress or decline, days remaining and discharge plans.
  • Evaluates and implements recommendations from established committees as they may pertain to the MDS assessment functions.
  • Establish and maintain open and effective lines of communication with all members of the care team who assess and plan care for the Medicare A Residents, to include physicians, nurses, rehabilitation therapists, social workers, dietitians, business office manager, nurse aides.
  • Supervise Assessment Nurse Coordinator.
  • Regular attendance, interaction with employees, and provide important and valuable service for the facility’s Residents is an integral requirement for this position to accomplish the work. In order to accomplish this, it is imperative for the person filling this position to be present and accessible to staff, Residents and Residents’ family members during normal business hours, and as needed for emergencies, unless a business related matter require the employee to be off the premises. It is anticipated that this position cannot be completed within 40 hours per week.
  • Resident preferences are followed when providing care and service (i.e. meal times, treatment times, type & time of bathing, sleep & wake times).
  • Maintain Resident privacy, values and right for decision making.
  • Immediately notify supervisor and Executive Director for any injury, illness, or if develop a condition, that might impact the aptitude to perform job functions.

Ancillary Duties and Responsibilities

  • Perform duties in compliance with established policies and procedures.
  • Consistently promote a safe environment for employees.
  • Serve on, participate in, and attend various other committees of the facility as required, and as appointed by the Executive Director.
  • Assist in insuring effective orientation of all new employees in accordance with the orientation manual regarding Medicare documentation, regulations, and other requirements.
  • When needed, supervise care delivered by nursing staff on the hall/unit.
  • Act as chairperson of PPS/Medicare Meeting.
  • Assure that appropriate restorative nurse training is completed and that appropriate restorative staff is competent in essential functions.
  • Attend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management, and blood-borne pathogens standard.
  • Attend and participate in advance directive in-service training programs for the staff and community.
  • Attend and participate in continuing education programs designed to help you keep abreast of changes in your profession, as well as maintain your license on a current status.
  • Conduct on-site assessments of hospitalized patients who have been referred as potential facility Medicare Residents.
  • Accept additional assignments within the scope of training, education, or experience that may be assigned from time to time by the Director of Nursing or Executive Director.

Resident Rights

  • Maintain the confidentiality of all Resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Ensure that the Resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are well established and maintained at all times.
  • Review Resident complaints and grievances and make written reports of action taken. Discuss such actions with Resident and family as appropriate.

Safety and Sanitation

  • Ensure that you and all facility personnel, Residents, visitors, etc., follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
  • Ensure that the building and grounds are maintained in a clean, safe, and sanitary manner as well as in good working order by reporting all concerns to the Executive Director.
  • Ensure that you and all facility personnel follow established regulations governing the use of labels and SDSs for hazardous chemicals.
  • Ensure that facility procedure manuals identify safety precautions and equipment to use when performing tasks that could cause bodily injury or exposure to a hazardous chemical or infectious waste.
  • Ensure that you and all facility personnel follow established ergonomics policies and procedures to prevent self-injury.
  • Ensure that you and all facility personnel performing tasks that involve potential exposure to blood, body fluids, or hazardous chemicals are appropriately trained prior to performing such tasks.

Unencumbered, active RN or LPN licensure in the state of permanent residence (must be from a state within the Nurse Licensure Compact).

  • Certified Nursing Assessment Coordinator or eligible.
  • One-year Long-Term Care experience preferred.
  • Current or recent experience with coordination, accuracy and completion of the RAI process.
  • Sincere desire to work with elders and others who have a limited capacity for self-care.
