Job description
Shiawassee Health & Wellness, an innovative Community Mental Health
organization, is seeking a clinician to provide outreach and in-home case
management services, as part of a treatment team, to children and families served
by Shiawassee Health and Wellness and Shiawassee Co. Health Department and
provide Wraparound services to the consumers of SHW children's community-based
program. Provides twenty-four-hour crisis intervention for SHW children's services on
a rotating, on-call basis, which may include Mobile Crisis Intervention and Response.
Coordinates mental health services with those of other community agencies to
prevent hospitalization and out-of-home placement and to promote individual, family
and community functioning.
Responsibilities and Duties
This clinician will provide intensive case management services to the parents and
children served by Shiawassee Health and Wellness and Shiawassee Co. Maternal
Infant Health Program. In addition to the case management role, this provider will
provide intensive, comprehensive, and individualized assistance to families and
children, who are being served by SHW, who have complex needs, which manifest
themselves as a serious emotional or behavioral disturbance and are at risk for out-
of-home placement. The clinician will coordinate and facilitate community services
and natural supports, and promote individual, family, and community functioning.
Lastly, the clinician will provide Wraparound services to select families served by
Shiawassee Health and Wellness
Qualifications and Skills
Education: Bachelor’s degree in social work from an accredited university, and
have appropriate certification, licensure or registration as required (LBSW or
LLBSW).
Experience: Two years of experience providing therapeutic case management
and/or out-reach services to children and families. Experience with infants and
toddlers preferred.
Other: Infant Mental Health certification and certification in Wraparound preferred.
Certification in PMTO, or TF-CBT, a plus. Must possess a valid driver license in
good standing as determined by the agency. Ability to work a flexible schedule,
including evenings as necessary, to meet the needs of consumers and the agency.
