Pierce County’s first Certified Community Behavioral Health Clinic; Comprehensive Life Resources is looking for a qualified LPN to join our MCIRT team in Spanaway, WA

Function: Provide services to the high 911 utilizing population in Pierce County affected by mental illness, substance abuse, self-neglect, progression of chronic illnesses, dementia/Alzheimer's, etc. Provide medication evaluation, first aid and address health concerns such as wound care, diabetes care, and activities of daily living. Assist with patient health education with a focus on self-management, prevention, and wellness based on client’s goals, and facilitate the community adjustment of consumers. Responsibilities:

• Administer medications and monitor medication compliance.
• Ensure follow-up on referrals and facilitate continuity of care.
• Complete nursing assessments and determine client healthcare needs and concerns.
• Provide general nursing care to include, medication evaluation, first aid, wound care, foot care, diabetes care and addressing other health concerns.
• Assist with gathering, documenting and monitor any lab tests needed according to agency procedures.
• Assist with patient health education with a focus on self-management, prevention and wellness based on client’s goals.
• Participate in treatment planning and nursing care planning
• Coordinate services to ensure continuity of care with community resources such as residential and community facilities, primary care providers, specialists and emergency medical professionals.
• Provide education and be a resource for facilities, professionals, and other treatment caregivers.
• Ensure that ethical and professional standards of practice are maintained.
• Maintain clinical records and provide reports in accordance with policies and procedures.
• Participate in meetings and activities as required.
• Coordinate activities with other team members and other staff.
• Demonstrate ongoing commitment to increasing competence and knowledge within the mental health and medical field.
• In addition to the above, perform other duties as assigned.

Qualifications:

• All new hires will be required to show proof of COVID-19 vaccination
• High school graduate or GED. LPN licensure.
• Experience in the provision of psychiatric services, including use of psychotropic medications preferred.
• Demonstrated ability to work with mentally ill individuals.
• Valid driver’s license when driving is necessary; proof of insurance.
