C.W. Community Health Center has an opening to join our behavioral health department in Charlotte, NC.

We are seeking a Licensed Clinical Social Worker (LCSW) to work with our Behavioral Health team as well as with our providers who work in a collaborative environment.

The ideal therapist will meet all of the requirements expected of a Licensed Clinical Social Worker (LCSW), including possessing the desire to make an impact on the lives of the patients and families we service.

LCSW Responsibilities:

• Consultations about clients
• Initial mental health evaluations resulting in realistic treatment plans
• Assessing clients using standardized assessment/screening tools, measures, etc.
• Provide brief, solution focused therapy
• Provide group psychoeducation/treatment related to chronic medical conditions;
• Responsible for care coordination and referral services;
• Crisis assessments/triage with appropriate interventions

• provision of other services related to wellness
• Documentation of scheduling and services provided in accordance with requirements

Qualifications and Skills

• Master’s Degree in Social Work
• 5+ years clinical social work experience
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Ability to make psychosocial assessments and develop and implement viable care plans
• Excellent listening and communication skills
• Bilingual preferred
• Experience with intimate partner violence counseling highly desired
• Experience with electronic health records (EHR)
• Athena EHR software experience highly desired
• Ability to provide after-hours service as needed
