Your Role in the Company

The Intake Coordinator is responsible for the coordination and processing of referrals and collection of Face-2-Face forms.

Qualifications:

• Licensed Practical Nurse currently licensed in the State of Florida, required
• Must possess strong communication skills
• Must be computer literate and possess fast, accurate data entry skills
• 1 year of home health experience, preferred

Responsibilities:

• Responsible for Intake/Referral process
• Responsible for collecting the Face to Face document upon intake
• Documents verbal orders received by physician
• Coordinate and Schedule Insurance Assessments
• Request records from doctor/facility as needed
• Coordinate SOC assessments
• Coordinate PT/OT/ST assessments when ordered upon SOC
• Track pending referrals/update pending referral board
• Track hospital holds/update hospital hold board
• Responsible for first point of contact with referral sources
• Maintain accurate patient information and demographics
• Create Start of Care packets
• Track Non Admissions
• Assist in other duties as assigned by Clinical Manager/ Administrator

Job Type: Full-time

Pay: $23.00 - $25.00 per hour
