POSITION SUMMARY

Provides prescribed medical treatment and personal care services, within the scope of the nursing assistant and as delegated by the registered nurse, to patients admitted at Riverwood Health Care Center. This position will also be required to work as a Health Unit Coordinator.

This position provides care for or assistance to patients of all ages, including newborns, pediatrics, adolescents, adults, and older adults.

POSITION QUALIFICATIONS

Education and Experience

GED or High School Diploma, and successful completion of CNA. Relatable experience related to working as a Health Unit Coordinator.

License/Certificates

Current NA Registration from the Minnesota State Board of Nursing. Current certification in BLS or ability to obtain during orientation.

ESSENTIAL DUTIES & RESPONSIBILITIES

CNA:

Provides safe patient care by maintaining a safe work environment and reporting near miss or actual incidents. Provides 1:1 Observation as assigned.

Manages the desk area by transferring calls, answering phones, answering call lights, and paging per request.

Documents in medical record, graphic sheet, and flow sheets according to Policy and Procedure, and reports significant changes to the RN.

Reports pertinent changes in patient condition to team members and charge nurse.

Provides basic patient care such as assistance with ADL’s, vital signs, glucose monitoring, etc.

Applies oxygen as directed by the RN.

Assists patients with nutritional needs by serving, setting up trays, and feeding as needed.

Monitors and records accurate intake and output.

Transports patients and assists patients with ambulation.

Assists with treatments such as ice packs, warm packs, etc.

Assists with collection and transport of lab specimens.

Restocks supplies as needed and per shift requirement. Manages clean and soiled utility room.

Safeguards security and confidentiality of data/information according to HIPPA guidelines.

Assist with discharge by gathering patient belongings and assisting patient to vehicle

Assumes responsibility for self-development and completion of mandatory educational requirements.

Enhances customer service by demonstrating polite, cooperative, and respectful behavior to internal and external customers.

Carry out duties in a safe manner and use safety devices provided to protect self and others. Report safety hazards as appropriate.

HUC:

Maintains a neat, clean, and orderly nurse’s station.

Manages the desk area, directs traffic, answers patient call lights and informs nursing of patient requests.

Accurately processes physician orders and is proficient in the electronic medical record.

Assembles charts for new patients and files charts at time of discharge.

Obtains charts from Health Information Services as requested.

At the direction of the RN, assists in preparing information for patient transfers to other facilities, and completes required forms.

Updates forms, files new forms, and discards out of date forms.

Requisition supplies designated by nursing staff. Orders office supplies. Obtains needed supplies from storeroom as requested

Schedule tests and makes appointments for lab, x-ray, PT, specialty clinic, etc.

Answer telephone and EMS Communications radio. Provides information or relays messages to patients and medical staff.

Maintain effective communication with the nursing staff, employees from other departments and other health care facilities.
