As a Housing Assistance Program Case Manage for Swope Health's Services you will be apart of a team of caregivers to provide care for the people who need it the most. The housing Assistance Program Case Manager is responsible for a variety of direct and indirect services to individuals referred to and/or receiving Housing Assistance Program Vouchers through the Missouri Department of Mental Health. The Housing Case Manager will practice Housing First models to decrease barriers and improve outcomes for individuals receiving a voucher for housing.

Successful Candidate will have:

  • Attend Continuum of Care meetings related to coordinated entry.
  • Complete the assessment required for housing vulnerability index scale.
  • Outreach individuals that may qualify and are not engaged with community mental health and/or substance use services.
  • Refer and transport (as needed) individuals seeking housing and participate in appointments to establish services.
  • Act as an advocate for individuals seeking housing.
  • Directly contact potential landlords and provide information on the voucher programs and make referrals to their units when open.
  • Complete data entry into the Homeless Management Information System as needed and review for referrals sent.
  • Complete SOAR certification and assist individuals with application for Social Security benefits.
  • Collaborate with community partners, Swope Health programs, and Department of Mental Health to ensure the best outcomes for housing voucher recipients.
  • Refer voucher recipients to support services for employment, housing readiness, mental health, substance use, and other benefits they may qualify to receive.
  • Assist individuals with determining their readiness for permanent housing and provide support with the move in process as needed.
  • Work independently to manage priorities and meet all deadlines.
  • Provide data to support future housing initiatives.
  • Participate in Performance Improvement activities related to Swope Health Quality Initiatives.
  • Perform other duties as assigned.

Education/Qualifications

  • Associates Degree in Human Services or or related field; OR 3 years of relevant case management experience in housing or community based mental health services. Experience with Shelter Plus Care and similar Housing Assistance Programs is preferred.
  • 1-2 years experience with individuals receiving psychiatric and/or substance use treatment services
  • Knowledge of Department of Mental Health and HUD housing program requirements and guidelines
  • Understanding of HMIS (Homeless Management Information System) and similar databases that provide service related information
  • Valid Driver’s License and reliable transportation
