Elara Caring is looking for a passionate Hospice Physician to join our elite team of healthcare professionals and make a difference, one patient at a time.

This position for a part-time independently contracted hospice physician estimating 10 hours per week with Elara Caring; a national hospice and homecare organization. This position will be based out of Topeka, KS. The Hospice Physician will participate in medical care duties over a census of hospice patients. They will have collaboration with hospice nursing and associated professional team, under the direction of a state-level hospice medical director with Elara.

Job Duties:

  • Conducts a biweekly hospice interdisciplinary team meeting consisting of hospice nurses, social workers, chaplains, and support staff, conducted at a branch hospice office.
  • Reviews and approve admissions to hospice and provides oversight of the plan of care and certificate of terminal illness.
  • Reviews medication programs and provide appropriate symptom management and adjustment of medications through prescriptive actions.
  • Willingness to participate in ongoing collaborative review of all hospice documentation to as part of quality assurance efforts.
  • Maintains ongoing review and education with updates to Medicare, State and Federal hospice regulations.
  • Provides assistance to hospice nursing for the management of urgent symptom needs
  • Participate in rotating after-hours call with other physicians to address symptom needs and new admissions.
  • Provides guidance and education to staff, patients and families in disease progression, prognosis, symptom management and advance care planning.
  • Desire to lead and drive team meetings with attention to all elements of hospice patient management including prognostication, medication management, support with difficult or complex pain, symptom, and grief scenarios.
  • Passion for education of hospice nurses, patients and families in the areas of end-of-life symptom management, advance care planning, effective communication.

Requirements:

  • Physician with Doctorate in Medicine (MD/DO)
  • Unrestricted State of Kansas physician license (MD/DO), DEA Controlled Substance License without restriction.
  • Unrestricted driver’s license and motor vehicle; able to travel and work in an office setting.
  • Ability to manage time; review and completion of documents, conducting of team meetings within a reasonable time range.
  • Agree with the organizational compliance program and code of ethics.

Preferred Qualifications:

  • Certified in Hospice and Palliative Care and/or 2+ years as a hospice physician with experience directing a hospice team.
  • Has experience in palliative communication, advance care planning, and grief support.
