We are looking for a Full-Time Registered Nurse (RN) for a Patient Care Manager (PCM) position in the Hospice Home Care setting. The pay and benefits for this Hospice Clinical Supervisor position are very generous. To qualify, the Registered Nurse must have at least two years' experience as a hospice RN or home health RN in the last 5 years, and one year of supervisory experience in any healthcare setting. Hospice management experience is a definite plus. The Patient Care Manager (PCM) is responsible for ensuring the delivery of high-quality, cost-effective end of life care to patient/families residing at home, Skilled Nursing Facilities (SNF) or Residential Care Facilities (RCF) through coordination of daily clinical operations and the supervision and support of assigned work groups providing that care. PCM ensures staffing coverage for direct contact with pts/families. The Patient Care Manager ensures accountability and compliance of team members to meet regulatory standards and company policies. The Hospice Clinical Supervisor is responsible for the direction and management of the interdisciplinary team. The PCM demonstrates sensitivity and compassion for the issues involved in serving patients and families facing a terminal illness, death and bereavement; clinical expertise in pain and symptom control, understanding of regulatory compliance, management and leadership skills and fiscal responsibility in the planning and delivery of services. REQUIREMENTS FOR THE HOSPICE CLINICAL SUPERVISOR POSITION: To qualify, the Registered Nurse must have at least two years experience as a hospice RN or home health RN in the last 5 years, and one year of supervisory experience in any healthcare setting. Hospice management experience is a definite plus. Must be a Registered Nurse. The registered nurse must have a license to practice as an RN in the state. The Registered Nurse must also either be a US citizen or have a green card- No exceptions. This company requires a COVID-19 vaccine. You may apply for the exemption.
