LPN will provide compassionate, direct patient care in the Valley. The successful candidate will have home health experience and perform LPN field work such as Wound Care, Wound Vac care, G tubes, Diabetic education, and CHF education. We are willing to train the right candidate.
Job Requirements:
- Must be 18 years of age or older
- Reliable - Personal transportation
- Valid AZ driver’s license, proof of insurance and registration
- Documents that show identity and eligibility to work in the U.S.
- Level 1 Fingerprint Clearance Card*
- CPR and First Aid Certification*
- Pass background check
- Negative Tuberculosis (TB) Screening (performed on 1st day of employment)
- Article 9 (DDD only)
- Physician's statement of fitness for duty

Salary: $40.00/visit with Experience
Estimated annual salary of $62,000 or higher is based on 30 visits per week.
Standard shift: 8 am - 5PM (Last patient scheduled at 4 PM)
Weekly schedule: Monday to Friday; Weekend rotation 1 weekend
Work Location: On the road
