The HIV/ID Clinical Pharmacist will integrate with the clinical team to provide patients and providers with therapeutic assessments for their disease management, and work with clinicians, care managres and all other clinical staff toward improving patient and practice outcomes. Assist with instititional and departmental regulatory surveys as needed. Adheres to Federal, State and local laws. Adheres to regulatory requirements regarding pharmacy and institutioal practice.

• Under direction, the HIV/ID Specialty Pharmacist's primary role will be integrating with the HIV/ID medical team and providing patients and staff with HIV treatment regimen education and adhereance tools
• Provides clinical pharmacy input during preparation of interdisciplinary care plans and follow up of patients
• Assista physicians in creating and managing the drug regimens of patients with chronic disease states

1. Possession of a valid NY State license to practice pharmacy.

2. Two (2) years of post-licensure experience; or

3. Completion of a post-graduate specialty residency or fellowship program; or

4. Current Pharmacy Board Certification.

• NYS pharmasist License in good standing
• Previous experience working with patients with HIV/AIDS
• Current HIV Specialty Pharmacist (AAHIVM) or expected to pursue certification
• Previous Infections Disease experience
• Pharm D Preferred / Board Certification
• Ability to multitask with good interpersonal, written and verbal skills
• Exellent analytical and problem solving abilities
• Must have basic computer skills
