YOUR ROLE

As a Health Coach with Evergreen Nephrology, you are responsible for a panel of patients and, in collaboration with other members of an interdisciplinary care team. You will assist patients to meet their goals and ensure they are taking the appropriate steps to manage their conditions and health. You engage with patients and inspire them to take an active role in their health by providing them with the tools necessary to make beneficial lifestyle choices and adopt lifelong healthy behaviors.

PRIMARY FUNCTIONS

A Health Coach's primary responsibilities center around establishing trusting, supportive, collaborative relationships with patients and their families and support network. You will build relationships with patients in a clinical setting, working alongside medical providers, other health coaches and a behavioral health specialist in a collaborative, empathetic team.

Reporting to the Market Operations Director, Health Coaches take on specific clinical, coaching, and operational responsibilities, highlighted below:


  • Capture relevant information about the patient's health and healthcare experience, take vitals, administer point-of-care testing, and perform standard age and condition-appropriate screening assessments.
  • Take responsibility (in partnership with providers) for the health of a population of patients and monitor and track whether those patients are up to date on preventive measures.
  • Learn about the chronic health conditions that span the patient population.
  • Provide patient education on chronic disease management.
  • Learn to employ confidence-promoting techniques both in person and in written communication to the patient and develop patient self-efficacy to better manage one's own health.
  • Leverage motivational interviewing techniques to help patients understand their chronic conditions and how they can better control them. Help patients realize their important goals and set plans to hit milestones.
  • Collaborate with other team members to facilitate group events and classes for patients.
  • Present on areas of interest/expertise in health and wellness to team members and patients.
  • Assist in setting up or troubleshooting technology in patient homes.

  • You have experience in a service environment, with demonstrated examples of service excellence.
  • You have exceptional interpersonal communication skills and can practice active listening. Your written communication skills are strong.
  • You may have clinical experience as an EMT, MA, CNA, or hold a Phlebotomy certification.
  • You are willing and able to complete the free Kidney Health Coach certification program from American Kidney Fund at start of employment.

  • Pay range for this role is 68,366 to 87,849, with exact pay determined based on experience, education, demand for role, and other role-specific criteria.
