JOB TITLE: Patient Engagement Specialist

JOB DESCRIPTION: The engagement team is the single most-important team in the organization. In this role, you will be the first point of contact for individuals in need. Sometimes, this is the first time they have reached out for help. Often times, they have reached out for help previously and received services that felt demeaning or judgmental. Your job in this role is to be as supportive, welcoming, and engaging as possible, while maintaining the highest standards of ethical and consistency in care. Your role is vital in setting the tone for care from YourPath. When people connect with us, they need to know immediately that they are safe and welcome, regardless of what might be happening.

WHAT WE DO: YourPath helps individuals needing substance use and mental health services, aimed at creating a wholistic approach to recovery. You will have the opportunity to learn our recovery platform specifically designed for YourPath. Our unique software allows us to serve our clients through an ecosystem platform that connects communities and providers with the individuals and families we serve, filling the gaps in treatment systems. Through our innovative software platform and care model we offer a turnkey solution to providing substance use disorder and mental health services.

QUALIFICATIONS & REQUIREMENTS

● Must be 18 years of age or older.

● Two years of freedom from problematic substance use problems.

● Current and valid drivers license.

● High School diploma or equivalent

● Must hold a current and valid Certified Peer Recovery Specialist (CPRS) credential issued by MN Certification Board

● A desire to work with individuals who need help with issues related to substance use, housing, mental health issues, poverty, individuals who have experienced trauma, racism, or other violence, and the ability and desire to treat all people with dignity and compassion.

● 1+ year working in a supportive healthcare setting, a high-energy fast-paced workplace, or an environment where consistency, customer service/engagement and timeliness were of priority.

● Ability to pass MN Department of Human Services background check as needed

PREFERRED QUALIFICATIONS

· Understanding of — or experience with — working with people with issues related to substance use, mental illness, health issues, poverty, or people with justice-involvement.

· Experience in healthcare or allied settings, use of an electronic medical record, or other communication governed by HIPAA.

RESPONSIBILITIES and DUTIES

· Using the YourPath technology platforms to connect individuals seeking help with effective care for a variety of issues related to recovery supports.

· Empower individuals who are seeking assistance by identifying their desires for treatment and working within a multidisciplinary team to identify suggested resources.

· Using videoconferencing platforms, scheduling systems, and other technology tools to maintain a high standard of care for people engaging with YourPath’s clinical, medical and recovery support services.

· Engage with external stakeholders, including third parties who are helping support individuals in care such as friends, family members, and professional and non-professional support services.

· Maintaining a consistent, engaging and professional standard of written engagement and over various methods of communication including email, text, and webchat.

· Complying with the various rules and ethics of providing care for vulnerable adults, including HIPAA, CFR 42, MN Statutes, and information security practices.

· Understanding and operating within the YourPath model of engagement for individuals and families affected by substance use issues and the various organizations and systems that serve them.

· Administrative and clerical tasks related to the care and referral for people seeking assistance from YourPath.

· Business development, outreach, marketing and engagement activities as assigned.

· Role model health communication and maintain a focus on self-care via honest communication with team.

· Attend relevant and required trainings, participate in team-building activities, and assist team as needed.

· Other duties as assigned.

REPORTING STRUCTURE

● The Engagement Specialist shall report to the Engagement Specialist Manager.

● Agency expects the highest standards of job performance from the Engagement Specialist as outlined on the performance review.
